Who we are

What we collect

• Information you send us. When you use a form on this site we receive your name, email address, phone number, the treatment you are interested in, your preferred timing and contact method, and any message you write.
• How you found us. If you arrive from an ad or link, the form also passes along campaign details such as the source and campaign name, the page you first landed on and the referring website.
• Usage data. If analytics are enabled, standard information such as pages viewed, device type and approximate location is collected through cookies.

Please do not send medical information

Website forms and email are not the place for medical history, diagnoses or photos. We will collect that privately at your visit, where it is protected as part of your medical record.

How we use it

• To respond to your request and schedule your visit, by the method you choose.
• To send the follow-up information you asked for. You can opt out at any time.
• To understand which pages and campaigns help people find us, so we can improve the site.
